IIRC... at one point Dutch had the P0408... and I had the P0108 about 4 years ago. (2016???)

The two times I have seen it both came down to the plastic of the EGR assembly cracking and leaking vacuum. I'm sure they all will eventually.

P0108 is fairly common on a Gen 2 LS at around 150k miles. P0408 also.

P0108 is not from the MAF, it's from the EGR assembly. Don't go aftermarket on the EGR. There are several posts on this.
