DCCV hose part number needed

I am hoping one of you guys have a part number I need it's the hose that goes from the dccv to the radiator hose. It has 2 different sizes at each end of the hose. The car is a 2003 LS V8 thanks yall.

I think it is 3W4Z-18472-AD.

I found a picture and I believe u might be right it looks like the part labeled 3 in this picture is it

Yes, to the best that I could tell. I know that Max got the number wrong way back when, so that makes me question what I found a little bit too.
