@Broseph here is a couple of pics. I had to jack the car up but i got you a couple of pics. The best i can tell the hose coming from the front went into the bottom and the hose thats in the back went to the top.

Ok with the codes you posted if you replaced the throttlebody with a good working unit and double checked all connections and coils are good then it sounds like you're pedal assembly maybe malfunctioning. Thats where i would go next.
@#Jeff504lsv8 i had ETC Failsafe problems with my throttlebody. I took that silver electric motor cover off thats attached to it and found those magnets thats glued to the inside of the cover were coming unglued and were sliding down and hitting that electric motor causing my 06 LS problems...

Yes a unhooked O2 sensor can cause this since its not relaying back correct info for the computer to make proper Air/Fuel trim adjustments. First i would try clearing the code and then completely reset your computer by unhooking the battery for 30 mins and see if it runs better before the code...
