Dreaded Engine Noise

When I first heard this I assumed it was the passenger side secondary timing chain tensioner. So I replaced that but the noise it still there. It seems to be coming from the back passenger side of the motor. It does quite up when RPMs increase. Any ideas as to what this could be? Really hoping it isn't the primary tensioner.

If it's not the primary guides +chains, it's out of time as well. Stop revving/running the motor like that. You ensured cam flats aligned on both sides when you had it open? Put the cam caps back in order? Torque down sequence followed? There are numerous videos on youtube with this symptom and also several threads on this form, some have had to do a complete on the timing.

The sound (IMO) is the chain slap on the inside of the block.

The flats were parallel before I removed the tensioner and I made sure to put it back that way. When I removed the caps I was extremely careful on keeping the bolts and caps in the right order and orientation. I hand tightened them, went to 53 in-lbs, then 90 more degrees.
 
sounds about right, may be that the chains are stretch though. not sure what else could be making that noise. Your picture of the tensioner doesn't look like it had failed completely just yet, sure it's got a crack in it but not all the way down and the chain runner is still attached plus not even worn down that badly at all.
 
Sounds kinda like a dropped exhaust valve to me, or one that isn't closing all the way. I would wonder if you don't have a cracked valve spring that is letting an exhaust valve close, but not all the way. Could also be a cracked exhaust header. Of course, I also have bad hearing, tinnitus and crappy laptop speakers contributing to my suggestions...
