Good morning friends...2006 LS with 93000 miles. Got the dreaded etc fail safe engine mode code. We changed all coils and plugs and still have the message plus these codes...p0132, p2106, p2135, p2196, and p0505. Any guidance and direction will be greatly appreciated...thanks in advance.

P0132 - O2 Sensor Circuit High Voltage Bank 1 Sensor 1 (forward sensor on passenger side)
P2196 - O2 Sensor Signal Biased/Stuck Rich Bank 1 Sensor 1 (forward sensor on passenger side)
P0505 - Idle Air Control System - consequence of the ETC problem.
P2106 - Throttle Actuator Control System - Forced Limited Power - consequence of the ETC problem.
P2135 - Throttle/Pedal Position Sensor/Switch "A"/"B" Voltage Correlation

Did you make sure the gaps on the plugs were all at 1.0 mm (Any that are more than 1.1mm will be a problem)? Did you use new Motorcraft coils? (Not from e-bay)

If yes to all above, maybe your gas pedal assembly really is bad...
Also, I'd fix that O2 sensor problem.

Well here is where we stand at this point...replaced O2 sensor bank 1 sensor 1 and replaced Throttle Position Sensor. ETC failsafe engine mode message still on and now we are down to only these two codes...p2106 and p2135. Further guidance and direction will be greatly appreciated...again!

I'm not familiar with the second gen, is there a tps on the top of the engine and then an accelerator pedal position sensor too? If so, did the right one get replaced?
Also, verify you did what Joegr said about plug gap and using quality coils. Misfires can cause false codes to appear. Did you delete all the codes and then they came back?

There is a tps on top which we replaced and thought it was the one, but apparently it wasn't. All the codes were deleted and the two I mentioned came up again.

Joe: is the "P2135 - Throttle/Pedal Position Sensor/Switch "A"/"B" Voltage Correlation" code saying that the PCM sees too much variance between what the pedal is saying and what the TPS is seeing, or am I reading that wrong?

There are two gas pedal position sensors in the gas pedal assembly. The code is saying that the two do not agree (in other words, one of the two has failed). The PCM doesn't know which one is the good and and which one is the bad one, so it can't trust either one. The TPS sensor on the throttle has nothing to do with this at all. The sensors are inside, in the gas pedal assembly. It's not meant to be repaired, only replaced.

So the gas pedal assembly has been replaced and these codes still remain...p2106 and p2135...any advice will be appreciated.

P2106 is only the indication that it's in limp mode. It should clear when you address other.

ISSUE:
Some vehicles built 11/29/2004 and prior may exhibit the wrench light on with both P2106 and P2135 diagnostic trouble codes (DTCs).
ACTION:
Replace the throttle position sensor (TPS) following the instruction sheet included in the TPS service kit.
Reprogram the powertrain control module (PCM) to the latest calibration using WDS release B36.11 and higher, or B37.2 and higher. This new calibration is not included in the B37 CD. Calibration files are also available at the website. CAUTION FAILURE TO FOLLOW THE TPS REMOVAL AND INSTALLATION PROCEDURE WILL RESULT IN DAMAGE TO THE TPS SCREWS.

Reflash to match ~ not sure if that helps but that's what I found.

Yeah, I think it down to coils/plugs, wiring, and the PCM itself then. (I am assuming that a new pedal assembly was used.)

Yea I fought this battle with etc fail safe engine mode, on my new to me 06 LS w/ ony 51k on it.

Lets check back about what you did so far, so that we can help you.

1. Did you replace all the Coils with new coils and boots/springs? What brand/from where?

2. Did you put in new plugs? What Brand. Did you Gap them? At what GAP?

3. Did you look in the speak plug wells for gunk, oil, rust or any other matter. Clean them.

4. Did you check the connectors, and look at the pins?

I'm thinking that it's 99% a Coil, Boot, Spring, Gunk, Plug, Plug Gap, Coil connector ignition problem.
